About the Role
As an Advanced Practice Provider, you will perform primary care services in home settings, conduct patient screenings and physical examinations, manage patient care plans, and collaborate with other healthcare professionals to ensure comprehensive care for patients. You will also engage patients in the WellBe program, address urgent care needs, and maintain accurate patient records.
About You
Required:
- Master's Degree in Nursing with a focus on Family Nurse Practitioner, Adult Nurse Practitioner, Geriatric Nurse Practitioner, or Adult-Geriatric Nurse Practitioner from an accredited program.
- Active, unencumbered Nurse Practitioner license in the state of Georgia.
- Current board certification with ANCC or AANP.
- BLS certification and an active NPI#.
- CPR certification.
- Active, unencumbered DEA licensure or eligibility to obtain.
- Valid driver's license and access to a car for patient home visits.
Preferred:
- 3+ years of Nurse Practitioner experience.
- Prior healthcare experience in roles such as medical assistant, certified nursing assistant, EMT/paramedic, lab assistant, or registered nurse.
- Experience in value-based care, home care, primary care, hospice, palliative care, or geriatrics.
- Experience with telehealth care delivery.
