AlignMed Partners is seeking an Advanced Practice Wound Care Provider to join their team in Langhorne, PA. The role involves providing acute and chronic wound care services to nursing center residents.
About the Role
As an Advanced Practice Wound Care Provider, you will report directly to the Director of Clinical Practice and collaborate with the Center Leadership Team and Medical Director for patient-specific medical care. Your responsibilities will include delivering high-quality wound care services and working towards improving healthcare outcomes for residents in skilled nursing and senior living facilities.
About You
Required:
- Graduate from an accredited School of Nursing program, licensed as a registered nurse.
- Certified as a Nurse Practitioner with a Master's Degree in Nursing and a concentration in Older Adult and/or Family Nurse Practitioner (FNP) or a Physician Assistant with PA National Certifying Exam (PANCE) certification.
- Wound Certification: CWS, WOCN, or appropriate wound certification, or actively pursuing wound certification with APN wound experience.
- Minimum of two years experience in wound care, working with the adult or geriatric population.
Preferred:
- Experience in a nursing center or long-term care environment.
- APRN with an unrestricted professional license and DEA for each state in which they practice.
