- High School Diploma or equivalent.
- Must be at least 18 years of age.
- Extremely organized with excellent communication and interpersonal skills.
- Professional, flexible, and able to work independently.
- Experience in community work and knowledge of community resources.
- Proficient in basic computer skills and Microsoft Office.
- Reliable transportation, valid driver's license, and current automobile insurance coverage.
- Associates or Bachelor's degree.
- Experience using an electronic health record (EHR).
- Bi-Lingual (Spanish Speaking).
