About the Role
About You
- Fluent in both English and Spanish.
- Current, unrestricted Multistate LPN or New Mexico LPN license.
- 3-5+ years' experience as a nurse.
- Proficiency with EPIC electronic health record and web-based applications.
- Excellent organizational and time management skills.
- Case Management or Chronic Disease Management experience.
- Certified Diabetes Educator.
- Experience with Motivational Interviewing or other behavior change communication techniques.
Benefits
- Compensation based on clinical encounters, with rates of $11.00 for the first 20-minute increment, $9.50 for the second, and $9.00 for the third.
- Bonus eligibility of $300 after three months of employment if performance expectations are met.
- Flexible scheduling with a minimum of 20 hours availability per week.
