About the Role
As a Care Coordinator, you will be a key component of the Mobile Health Clinic team, working to coordinate care for clients in underserved areas. Your responsibilities will include working directly with clients and their families to secure necessary services, endorsing integrated care principles, and building rapport through empathic listening and support. You will also be part of a multidisciplinary team that travels to various locations to provide essential health services.
About You
Required:
- Bachelor's degree in the social sciences or a mental health field (e.g. counseling, psychology, social work, vocational rehabilitation) or Bachelor's degree transcript including 15+ credit hours of related courses.
- Must maintain a valid TN driver's license and a safe driving record.
- Excellent time management skills.
Preferred:
- Experience working with individuals with severe and persistent mental illness.
- Desire to provide services to residents of rural communities.
