About the Role
As a Care Coordinator I, you will collaborate with members, their families, and care teams to ensure appropriate care and address healthcare access barriers. Your responsibilities will include managing annual support plans, ensuring members' health and safety needs are met, maintaining regular contact with members, and documenting activities in the EMR. You will also promote continuous learning and compliance with relevant regulations.
About You
Required:
- Minimum completion of 2 years of college-level coursework in social work, psychology, sociology, or a health-related field.
- Minimum of 1 year of experience in care coordination and beginner-level experience with Microsoft Excel, Outlook, and Word.
Preferred:
- Bachelor's degree in social work, psychology, sociology, or a health-related field.
- 3-5 years of experience in care coordination within managed care and/or the Arkansas PASSE.
- Licensed Practical Nurse (LPN) certification.
