About the Role
As a Care Coordinator, you will work collaboratively with physicians, patients, families, and healthcare staff to coordinate healthcare programs, develop individualized care plans, educate patients on their healthcare options, create goals, monitor progress, recruit and train staff, and create schedules.
About You
Required:
- Strong problem-solving and organizational skills.
- Ability to manage multiple projects or tasks and prioritize appropriately.
- Ability to work in fast-paced situations and make sound decisions quickly.
- Excellent interpersonal skills and high level of compassion.
- Strong verbal and written communication skills.
Preferred:
- Previous experience as a Care Coordinator or in a similar position.
- Certification as a medical assistant.
