Summary
Integrated Services for Behavioral Health is seeking a Care Coordinator to join their team in Gallipolis, OH. The role involves service linkage and care coordination for children, youth, and families with significant behavioral health needs.
About the Role
As a Care Coordinator, you will engage with families to identify care coordination needs, work collaboratively with community agencies, and ensure that children, youth, and families have a voice in their care. Your responsibilities will include coordinating family-based services, maintaining documentation, and participating in program evaluations and training.
About You
Required:
- Experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field.
- Three years with a high school diploma or equivalent; or two years with an associate degree or bachelor's degree; or one year with a master's degree or higher.
- Valid Driver's License required.
Preferred:
- Knowledge and experience in Hi-Fidelity Wraparound.
- Two years of experience in a coordinated supportive services or care coordination role.
- Experience working with people with autism spectrum disorders and developmental disabilities.
- Experience in family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development.
