About the Role
As a Care Coordinator- Registered Nurse, you will collaborate with the Advanced Prostate Cancer Care team to enhance the APC program, facilitating patient navigation through the APC Care Pathway. Your responsibilities include acting as an advocate and primary contact for patients, assisting in the development of policies and protocols for prostate cancer care, planning treatments across cancer programs, managing IV access, and coordinating consultations with specialty physicians.
About You
Required:
- Minimum of 2 years' experience working with cancer patients in a clinical environment.
- Valid Registered Nurse license in Minnesota.
- Proficiency in MS Office Suite, EMR, Practice Management, and Lab Information Systems.
- Strong organizational skills and attention to detail.
Preferred:
- Experience in oncology care.
- Excellent interpersonal communication and empathetic listening abilities.
