About the Role
As a Care Coordinator Supervisor, you will oversee the activities of up to 7 Care Coordinators, ensuring that all care management activities are aligned with organizational policies. You will be responsible for supervising cases, managing client records, ensuring billing requirements are met, and providing training to staff. Additionally, you will represent the Health Connections program in the community and participate in interdisciplinary team meetings.
About You
Required:
- Master's degree (LMSW/LCSW, LMHC, or related field) plus 2 years of supervisory experience.
- Experience with Care Management.
Preferred:
- Excellent organizational and time management skills.
- Strong verbal and written communication skills.
- Bilingual Spanish.
