Summary
Children's Home of Jefferson County is seeking a Care Coordinator to join their team in Watertown, NY. The role involves providing comprehensive and person-centered care coordination and management services for members experiencing emotional and/or behavioral problems or complex chronic conditions.
About the Role
As a Care Coordinator, you will conduct strength-based assessments, identify and submit referrals for needed services, and provide advocacy and support to individual members. You will collaborate with members and their care teams to develop individualized service plans, ensuring the delivery of high-intensity care management services and facilitating access to community resources. Your responsibilities will also include maintaining accurate records and collaborating with care team members to schedule and facilitate team meetings.
About You
Required:
- An Associate’s Degree in psychology, sociology, human services, or a related field is required.
- Strong organizational abilities and excellent oral and written communication skills.
Preferred:
- A Bachelor’s Degree in a related field is strongly preferred.
- Licensure or certification in a related field such as LPN or CASAC may be substituted for an Associate's Degree.
