About the Role
As a Care Management Nurse, you will provide clinical care management, including case management, disease management, and care coordination, to meet members' healthcare needs. You will oversee a collaborative process to assess, plan, implement, coordinate, monitor, and evaluate care, ensuring a person-focused healthcare experience.
About You
Required:
- Associate or Bachelor’s Degree in Nursing or related field.
- 3 years of case management, utilization management, disease management, auditing or retrospective review experience.
- Must have licensure or certification in a health or human services discipline that allows independent assessment.
Preferred:
- Bachelor’s degree (or higher) in a health or human services-related field.
- Registered nurse (RN) license for medical care management.
