About the Role
As a Care Management Supervisor, you will supervise care management teams, ensure program protocols are followed, conduct planning sessions, coordinate consumer assignments, address team concerns, provide support in crisis situations, and ensure efficient transitions for consumers. You will also be responsible for performance appraisals, documentation monitoring, and community relations.
About You
Required:
- Bachelor's degree in a health-related field with a minimum of 6 months care management team leader experience or 1+ years of care management tier experience.
- Valid Tennessee Driver's License.
- Acceptable Motor Vehicle Report (MVR).
- Acceptable Criminal Background Investigation.
- Personal Automobile Insurance.
- Transportation that seats 4 people.
- Cell Phone.
Preferred:
- Master's degree (M.A., M.S.) in a health-related field.
- Experience in a mental health setting.
