About the Role
As a Care Manager - Behavioral Health, you will evaluate member needs through phone or in-home visits, develop ongoing care plans, coordinate with community resources, monitor care plans, and provide education to members and their families. This hybrid position requires face-to-face visits in New Castle County.
About You
Required:
- Master's degree in Behavioral Health or Social Work or a Degree from an Accredited School of Nursing.
- 2 – 4 years of related experience.
- Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW) or RN based on state contract requirements.
Preferred:
- Experience in care management for mental health and substance use disorders.
- Strong communication and interpersonal skills.
