About the Role
As a Care Transitions Coordinator, you will be responsible for coordinating patient admissions, conducting on-site visits to assess patient needs, and maintaining accurate records of patient interactions. You will also develop relationships with hospitals and physician offices to generate leads and achieve admissions and sales goals.
About You
Required:
- Current RN or LPN license specific to the state(s) you are assigned to work.
- At least one year of experience in a healthcare related business development/outreach role.
Preferred:
- Bachelor's degree in a relevant field.
- Experience in cold calling customers in pharmaceuticals, medical device sales, or home health.
