About the Role
As a CareCoach Connect Community Health Worker, you will engage with members face-to-face or over the phone to discuss their healthcare needs, provide education on community resources, and encourage participation in health-related programs. You will work closely with clinical and administrative staff to assess patient needs and ensure effective outreach services.
About You
Required:
- Ability to attain CHW certification within 1 year of hire
- 2+ years of experience within the community
- Texas driver’s license and access to reliable transportation
- Ability to travel up to 90% around the designated areas to meet with members
Preferred:
- High School diploma/GED
- Current Certification as Community Health Worker by the State of Texas
- 2+ years of experience in resident-driven service delivery
- Bilingual (English/Spanish)
