About the Role
As a Case Management Assistant, you will assist Nurse Case Managers and Social Services staff by conducting outreach, organizing information, coordinating appointments and referrals, and documenting activities. Your focus will be on ensuring timely communication and access to services for members, supporting member engagement activities, and improving health outcomes. You will also gather clinical information, assist with care coordination, and participate in interdisciplinary care team meetings.
About You
Required:
- High School diploma or GED required.
- Minimum of 3 years of experience in a healthcare or community health setting.
Preferred:
- Knowledge of case management processes and regulatory requirements for Medi-Cal, Medicare, and commercial programs.
- Experience in a managed care environment (IPA, HMO, or health plan) preferred.
- Medical Assistant or Community Health Worker certification preferred.
