Summary

About the Role
As a Case Management Associate 1, you will be responsible for creating and updating participant plans, determining eligibility for supportive services, monitoring client progress, and maintaining communication with clients and relevant organizations. You will also ensure compliance with program regulations and maintain up-to-date knowledge of community resources.
About You
Required:
- High School Diploma or equivalent required. Degree preferred.
- 2+ years of relevant work experience.
- Ability to work with and relate to participants and demonstrate active listening skills.
- Excellent customer relation techniques, and superior verbal and communications skills.
Preferred:
- Experience in case management or related field.
- Familiarity with specialized databases and software systems.
