Firelands Health is seeking a Case Manager: FT to join their team in Fremont, OH. The role involves providing community-based services to youth and adults with mental health and substance use disorders.
About the Role
As a Case Manager, you will be responsible for delivering mental health and substance use related case management services, including ongoing assessment of needs, crisis support, and stabilization activities. You will coordinate treatment plans, provide advocacy and outreach, and ensure compliance with healthcare standards. Your work will involve promoting patient recovery through collaborative treatment planning and engaging with community resources.
About You
Required:
A Bachelor degree or higher in social services related field.
Experience working with persons with mental health or substance use problems preferred.
Heart Saver, Aegis Crisis Prevention, and First Aid certification within 6 months of hire.
Ability to effectively understand and communicate in both written and oral form.
Preferred:
Punctual attendance at assigned work location is required.
Ability to be mobile and possess a valid State of Ohio motor vehicle operator’s license.
Ability to comply with OSHA regulations and CDC standard and transmission-based precaution recommendations.
