Summary
City of Fremont is seeking a Case Manager (Family Resource Center) to join their Human Services Department in Fremont, CA. The role involves performing professional and clinical social work with individuals and families who have complex social and medical needs.
About the Role
As a Case Manager, you will manage a caseload of clients, conduct comprehensive assessments, develop and implement care plans, and provide supportive counseling and advocacy. You will also coordinate services to help clients achieve self-sufficiency and family stabilization, prepare reports, and may supervise intern training.
About You
Required:
- Master's Degree in social work, psychology, counseling, sociology, nursing, gerontology, or a related field, and one year of case work experience, or a BA in the same fields with three years of social service experience, including one year with children, youths, or senior citizens.
- Possession of a valid Class C California Driver's License.
Preferred:
- Ability to speak Spanish, Chinese, or other Asian languages.
