Children's Rescue Fund is seeking a Case Manager for their Icahn House location in Bronx, NY. The role involves providing direct case management services to assist families in transitioning to permanent housing.
About the Role
As a Case Manager, you will be responsible for conducting assessments, providing counseling, crisis intervention, and advocacy to ensure effective social service support plans are in place. You will utilize the DHS Client Assistance and Re-housing Enterprise System (CARES) for intake assessments and service plans, coordinate services for families, and facilitate group sessions to enhance life skills. Your role will also involve maintaining relationships with clients and ensuring compliance with relevant regulations.
About You
Required:
At least an Associate’s degree in Social Work, Sociology, Psychology, Counseling, Criminal Justice, or Human Services.
At least one year of experience in social services, counseling, crisis intervention, family services, preventive services, or housing services.
Experience facilitating groups for homeless adults and specialized knowledge of substance abuse issues.
Preferred:
Bachelor’s degree in a relevant field.
Bilingual language ability.
Excellent written and verbal communication skills.
