About the Role
As a Case Manager (Service Coordinator), you will establish positive working relationships with individuals served, their teams, and external stakeholders. Your responsibilities will include developing and monitoring Person-Centered Plans, advocating for individuals, providing education on service coordination, and ensuring compliance with state-mandated requirements. You will also engage in person-centered planning and assist individuals in accessing necessary resources and services.
About You
Required:
- A bachelor's degree from an accredited education program in a human services field or a non-human services field with two years of experience in a human services field.
- Strong communication and interpersonal skills.
Preferred:
- Experience working with people with developmental disabilities.
- Knowledge of case management practices and state regulations.
