About the Role
As a Case Manager (Supports Planner), you will be responsible for establishing positive working relationships with individuals supported, their support systems, and external stakeholders. Your primary duties will include developing and monitoring Person-Centered Plans of Service, advocating for individuals, coordinating resources, and ensuring compliance with state mandates. You will also engage in person-centered planning and provide education to empower individuals to achieve independence.
About You
Required:
- Bachelor's or master's degree in Psychology, Sociology, Social Work, Nursing, Counseling, or Human Services Administration.
- Valid Driver's License and an insured vehicle.
Preferred:
- Experience in working with people with disabilities.
- Experience with person-centered planning.
