Summary
New River Valley Community Services is seeking a Case Manager - Medical Services (Jail-MAT) to join their team in Blacksburg, VA. The role involves providing case management for individuals with mental illness and substance use disorders.
About the Role
As a Case Manager, you will coordinate services and resources for clients to support their basic needs and help them function optimally in the community and/or Jail. You will work as part of the SA Team to assist clients enrolled in MAT programs, provide discharge services, and engage clients in appropriate follow-up services. This position is crucial in helping individuals improve their daily functioning and community involvement.
About You
Required:
- Bachelor's Degree in Education, Social Work, Psychology, or a Human Services related field defined by the VA Board of Counseling.
- Registration as a QMHP or eligibility is required.
Preferred:
- At least one year experience in a setting working with individuals with disabilities, mental health needs, or substance use.
- Previous Case Management experience.
