Summary
ITXL is seeking a Case Manager to join their team at the South King County Clinic in Renton, WA. The role involves providing outreach, engagement, and intensive case management services to individuals experiencing homelessness and substance use disorders.
About the Role
As a Case Manager, you will be part of the Behavioral Health Team, responsible for direct support to individuals facing homelessness and co-occurring disorders. Your duties will include care coordination, community resource linkage, and advocating for systemic change. You will engage with participants through outreach activities, conduct needs assessments, and assist in creating Individualized Service Plans. Additionally, you will provide housing support, coordinate medical and behavioral health services, and advocate for participants' rights within various systems.
About You
Required:
- High School Diploma required; associate or bachelor's degree in social work, human services, psychology, or related field preferred.
- At least two years of experience working with individuals experiencing homelessness, substance use, or behavioral health challenges.
Preferred:
- Experience in harm reduction and outreach programs.
- Strong interpersonal skills and verbal/written communication skills.
