Summary
Project Renewal is seeking a Case Manager to join their team at the Barbara Kleiman Men’s Shelter in Brooklyn, NY. The role involves providing assessments, case management, and support services to homeless men, including those with mental illness and/or substance use disorders.
About the Role
As a Case Manager, you will be responsible for conducting intake assessments, creating and managing Independent Living Plans for clients, and ensuring accurate documentation in compliance with oversight guidelines. You will work collaboratively with clients to help them achieve personal goals, secure housing, and access necessary services. The position requires physical activity, including navigating the facility and assisting clients with appointments.
About You
Required:
- Bachelor’s degree in social work, public administration, psychology, education, sociology, occupational or physical therapy, or related human services fields with a minimum of two (2) years of experience.
- An Associate’s degree in the aforementioned fields with three (3) years of experience.
- A High School Diploma or GED equivalent with four (4) years of experience.
Preferred:
- Bilingual or multilingual skills.
- Demonstrated leadership ability to manage and motivate staff.
- Excellent oral, writing, and listening communication skills.
- Strong organizational skills to work under pressure and meet deadlines.
- Ability to interface effectively with clients and staff at all levels.
- Knowledge of case management software and proficiency in Microsoft Office Suite.
