About the Role
As a Case Manager, you will be responsible for delivering safe, timely, effective, and client-centered care. Your duties will include interacting with clients and their families, tracking progress, completing necessary paperwork, assessing motivational and psychosocial issues, creating service plans, and assisting in case closures. Flexibility in scheduling, including evenings and weekends, is required to provide individualized interventions.
About You
Required:
- Experience as an STNA, CNA, or nursing background within a nursing home setting.
- Minimum of 3 years’ experience working with older adults in a professional and supervisory capacity.
- Valid driver’s license and automobile insurance with minimum state liability amounts.
Preferred:
- Experience in social work, counseling, or human services.
- Demonstrated experience in engaging adults in therapeutically oriented activities.
- Strong organizational, interpersonal skills, judgment, flexibility, commitment, and internal drive/motivation.
