JASA is seeking a Case Manager-CIAA NORC to join their team in Brooklyn, NY. The role involves providing in-home and in-office assessments for older adult clients and developing comprehensive case plans to address their social and emotional needs.
About the Role
As a Case Manager-CIAA NORC, you will be responsible for assessing the social and emotional needs of older adult clients and their families, developing and implementing case plans, providing counseling, securing benefits, and coordinating services such as transportation and homecare. You will also conduct group counseling, represent the agency at community events, and maintain case records.
About You
Required:
Graduate of an Accredited College or University with BSW or BA/BS degree, or equivalent.
Good writing/record keeping skills.
Ability to read, write, speak, and understand English.
Must be computer literate, including ability to learn and use program-specific software.
Ability to travel to clients' homes using mass transit.
Preferred:
Knowledge of Spanish may be helpful.
Understanding of the roles of race, religion, ethnicity, sexual preference, and individual values as they relate to serving clients.
