About the Role
As a Case Manager Entry, you will be responsible for conducting assessments of participant strengths and needs, providing coaching for social support, coordinating care and case management, advocating for participants, and engaging in outreach activities. You will work on-site to support individuals and families residing in interim and permanent housing, helping them navigate the health and social service systems.
About You
Required:
- Must be able to pass a fingerprint clearance and background check.
- 6 months of social services case-related experience, or an Associate’s Degree in a social services related field.
Preferred:
- Current California driver's license and clean driving record.
- Case management training within 6 months of hire.
