About the Role
As a Case Manager, you will be responsible for performing intakes on prospective clients, delivering meals, conducting recertification visits, and executing reassurance visits. You will also deliver essential items to clients, perform well-checks, and assist with meal delivery when necessary. The position requires maintaining confidentiality, supporting the agency's mission, and contributing to special projects.
About You
Required:
- Four-year university degree in Social Work, Human Services, Psychology, Sociology, or related field.
- Physically able to lift and carry objects of 25 pounds.
- Physically able to enter/exit your vehicle multiple times a day, walk up and down stairs, reach, stretch, bend, and sit for multiple hours at a time.
Preferred:
- Experience in social work or case management.
Benefits
- Hourly wage of $17-$20.
- Full-time employment with a structured schedule from 8:00 AM to 4:30 PM, Monday to Friday.
