About the Role
As a Case Manager, you will assess patients for appropriate care levels, coordinate transfers between facilities, and ensure effective communication with healthcare providers. You will also identify post-hospital care needs, provide clinical consultations, and maintain patient confidentiality while adhering to insurance and regulatory requirements.
About You
Required:
- Graduate of an accredited RN Program.
- Minimum two years’ experience as a RN in an acute care setting.
- Ability to communicate effectively verbally and in writing.
- Must be able to work in a union environment.
- AHA BLS Card.
- Current Los Angeles County Fire Card required (within 30 days of employment).
Preferred:
- Completion of Case Management Course.
- Minimum two years as a Case Manager.
