Summary

About the Role
As a Case Manager I/II, you will work under the direction of a case management supervisor to provide services to individuals with intellectual and/or developmental disabilities. Your responsibilities will include obtaining in-depth social histories, developing community resources to meet consumer needs, and serving as QIDP while providing monthly program monitoring. You will also need to comply with Federal Home and Community Based Waiver regulations and state policy guidelines.
About You
Required:
- Bachelor's degree in a human service field or a valid permanent Oklahoma Nursing license with one year of experience working with individuals with developmental and/or intellectual disabilities.
- Completion of the Case Manager Academy Training Program.
Preferred:
- One year of professional experience in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, program coordination, nursing, or a closely related field.
