Telecare Corporation is seeking a Case Manager III - Mental Health to join their team in Oakland, CA. The role involves serving as a primary support and case manager for individuals with mental illness and co-occurring disorders, assisting them to live independently in their community.
About the Role
As a Case Manager III, you will work under direct supervision to provide case management services to individuals with mental health challenges. Your responsibilities will include helping clients access resources, driving them to appointments, and ensuring they receive the support needed to thrive in their chosen community. This position requires a higher level of education and experience compared to a Case Manager II.
About You
Required:
Bachelor’s degree in Social Services or Human Services and 1 year of experience, or High School Graduation/G.E.D. equivalent and 4 years of direct services in Mental Health or community services.
Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
Willingness to use your personal vehicle to drive clients to appointments and groups.
Preferred:
Experience in case management or mental health services.
Associate's degree may be substituted for two years of experience.
