About the Role
As a Case Manager, you will be responsible for receiving and responding to incoming calls from referral sources, completing pre-screening assessments, overseeing field staff, consulting with third-party representatives regarding client benefits, and maintaining relationships with referral sources. You will also monitor referral satisfaction levels and ensure compliance with company policies and regulatory mandates.
About You
Required:
- Bachelor's degree in a human services field or nursing field.
- Valid driver's license.
- Two years of experience with case management or related discipline.
- Ability to communicate both verbally and in writing.
