About the Role
As a Case Manager RN- Field, you will be responsible for conducting face-to-face visits with members in their homes and facilities, performing assessments, providing education on conditions, and collaborating with interdisciplinary teams to improve health outcomes. You will spend at least 75% of your time in the field, focusing on high-risk members and ensuring they receive necessary services to reduce readmissions.
About You
Required:
- Ability to get licensed in other states as needed.
- 3+ years of RN experience.
- Renal, Chronic Kidney Disease or Dialysis Care experience as a main focus of your job.
- Computer proficiency, including strong data entry and MS Office skills.
- Reside in a location that can receive a high-speed internet connection.
Preferred:
- BSN or higher level of education.
- Certified Case Manager (CCM).
- Diabetic Educator experience.
- Field-based experience going into homes.
- Telephonic case management experience.
- Experience working within a managed care environment.
