About the Role

As a Case Manager Social Worker, you will handle a caseload of patients, communicate with external payers, and work closely with the interdisciplinary team to monitor utilization and length of stay. You will educate patients and caregivers on the continuum of care, facilitate discharge planning, and provide referrals to community resources. Your responsibilities will also include coordinating with financial services and admissions to ensure smooth transitions between levels of care.
