About the Role
As a Certified Nurse Practitioner, you will perform physical exams, order and interpret diagnostic tests, manage acute and chronic conditions, prescribe medications, collaborate with healthcare staff, and educate patients and families on health conditions. This position requires travel between multiple facilities in the Kankakee, Bradley, and Wilmington areas.
About You
Required:
- NP License (or eligibility to obtain)
- Passion for geriatrics, internal medicine, or primary care
- Strong team player with excellent communication skills
- Self-motivated with a patient-first approach
Preferred:
- Experience in post-acute care or skilled nursing
- New graduates are welcome – training and mentorship provided
