Summary
Carebridge EAP is seeking a CFSP Managed Care Coordinator for Region 2 to join their team in Asheville, NC. The role involves managing individual service plans for Medicaid-enrolled children and families within the CFSP program.
About the Role
As a CFSP Managed Care Coordinator, you will provide integrated whole-person care management, coordinate across various health services, and collaborate with multidisciplinary teams to support the health care needs of children and families. Your responsibilities will include conducting assessments, setting goals, and identifying members who may benefit from expanded services, all while ensuring trauma-informed care and compliance with state regulations.
About You
Required:
- BA/BS degree and a minimum of 1 year of experience working directly with people related to the specific program population or other related community-based organizations.
Preferred:
- Must reside in North Carolina.
- BA/BS degree preferred in a field related to health, psychology, sociology, social work, nursing or another relevant human services area.
- Two (2) years of experience working directly with individuals served by the child welfare system is preferred.
