As a Chronic Care Manager, you will work with multidisciplinary teams to evaluate patient health statuses, develop care plans, and ensure effective communication with patients regarding their care. You will also connect patients to necessary resources, document encounters in the EHR, and participate in pre-visit planning with providers.
About You
Required:
- Graduation from an accredited program for Medical Assistants or an accredited school of nursing.
- Certification or registration from an approved certifying organization for Medical Assistants.
- Possession of a State Practical Nurse license or a current State of Iowa Registered Nurses license.
- Current Basic Life Support certification for Health Care Providers.
- Minimum of 5 years in a multi-specialty clinic or similar organization.
Preferred:
- Experience with population health, care coordination, and/or disease management programs.
- Certified Health Coach.
