About the Role
As a Chronic Care Manager, you will perform telephonic encounters with patients, develop detailed care plans, and manage a patient panel of at least 100 patients per month. You will work part-time from home, setting your own hours while complying with HIPAA regulations. The position starts as an Independent Contractor with the potential for full-time employment and benefits.
About You
Required:
- Graduate from an accredited School of Nursing (LPN, LVN, RN, BSN, etc.).
- Current COMPACT license to practice as an RN/LVN/LPN in the state of residence with no disciplinary actions.
- Minimum of two years of clinical experience in Med/Surg, Case Management, or home health care.
- Hands-on experience with Electronic Medical Records and proficiency in Microsoft Office 365, Teams, and Excel.
- Ability to exercise initiative, judgment, organization, time-management, problem-solving, and decision-making skills.
- Excellent verbal, written, and listening skills.
Preferred:
- Ability to quickly recognize condition-related warning signs.
- Organized and thorough documentation skills.
- Self-directed with demonstrated time management skills.
- Exemplary phone etiquette and clear diction.
- Commitment to excellence in patient care and customer service.
