About the Role
As a Client Care Coordinator, you will coordinate care, treatment, and services within multiple health care systems, create proactive care plans, assist clients in securing medical care, and maintain accurate progress notes. You will engage with clients persistently to assess their needs and collaborate with external stakeholders to ensure continuity of care.
About You
Required:
- Bachelor's Degree and a minimum of two years’ experience working with clients in mental health, substance use, trauma, violence risk, homelessness, or criminal justice involvement.
- Demonstrated success in assessing, executing, and advocating care plans.
Preferred:
- Experience in a multidisciplinary setting and field-based clinical work.
- Fluency in Spanish.
- Certifications such as CASAC, CASAC-T, or other SU or MH certifications.
