About the Role
As a Clinical Care Coordinator, you will work at Ridgefield and Danbury Primary Care locations, conducting assessments and collaborating with patients, providers, and multidisciplinary teams to create and monitor treatment plans. You will also assist in resource management and provide education to patients and families regarding care options and community resources.
About You
Required:
- Master's degree in Social Work from an accredited school.
- Minimum of 3 years' experience.
- Current CT Licensed Clinical Social Worker (LCSW) in NY & CT.
Preferred:
- Excellent working knowledge of behavioral medicine and evidence-based treatments.
- Experience in hospital case-management or crisis intervention.
- Strong communication skills and ability to work in a fast-paced primary care environment.
