Summary
Outcomes® is seeking a Clinical MTM Pharmacist-PRN to provide patient-centered Medication Therapy Management services aimed at optimizing medication therapy and improving clinical outcomes.
About the Role
As a Clinical MTM Pharmacist, you will deliver comprehensive medication reviews, conduct patient outreach, document patient interactions, and communicate clinical recommendations to prescribers. You will assess medication therapy for appropriateness and safety, develop evidence-based interventions, and collaborate with interdisciplinary teams to establish individualized therapy goals.
About You
Required:
- Minimum of one year of work experience.
- Bachelor’s Degree in Pharmacy or PharmD from an ACPE-accredited college.
- Current license to practice pharmacy in the applicable state(s).
- Ability to obtain pharmacist licensure in multiple states if requested.
- Completion of an MTM certificate course.
Preferred:
- Board certification in Ambulatory Care, Geriatrics, Pharmacotherapy, or another ACCP-recognized certification.
- Completion of an ASHP-accredited pharmacy residency and/or fellowship.
- Pharmacist licensure in multiple states.
- Bilingual fluency in English and Spanish, Mandarin, or another language.
- Motivational interviewing skills.
