About the Role
About You
- Graduate of an accredited school of Pharmacy and licensed in the State of Ohio.
- Experience in pharmacy practice, preferably in an ambulatory setting.
- Experience in program development, implementation, and evaluation.
- Training or experience in population health concepts and practices.
- PGY-1 Residency or Equivalent Clinical Experience.
- Experience in working in and leading interdisciplinary teams to improve patient outcomes.
