About the Role
As a Clinical Pharmacist, you will assess patients for home care suitability, design and monitor pharmacy care plans, provide medication education, and ensure compliance with safety and regulatory standards. You will also manage medication administration equipment and respond to drug information requests from healthcare providers.
About You
Required:
- Graduate of an accredited School of Pharmacy.
- Current, active Pharmacy license as granted by the State Board of Pharmacy.
- At least one year of experience in sterile IV admixture procedures or equivalent experience.
Preferred:
- Completion of an ASHP accredited Pharmacy Residency Program.
- Experience in home care, hospital, or other clean room settings.
