Summary
Upstate Caring Partners is seeking a Community Habilitation Specialist to join their team in Utica, NY. The role involves the development, implementation, and monitoring of individualized family supports and staff action plans.
About the Role
As a Community Habilitation Specialist, you will be responsible for completing initial waiver service intakes, identifying individual habilitation needs, developing and monitoring Staff Action Plans, promoting independence through person-centered programming, and ensuring compliance with regulations. You will also supervise direct support staff, maintain documentation, and participate in leadership activities within the Community Services team.
About You
Required:
- Associate’s Degree in human services or related field (e.g., psychology, sociology).
- Minimum of 12 months experience working with people with developmental disabilities.
- Valid NYS driver's license and reliable transportation.
Preferred:
- Experience in developing individualized support plans.
- Strong communication and interpersonal skills.
