About the Role
As a Community Health Worker, Housing Lead, you will be responsible for coordinating client-centered care, supporting clients in housing transition and tenancy sustainability, and serving as a housing resource for other Community Health Workers. Your daily tasks will include managing an active caseload, conducting tenant screenings, developing individualized housing support plans, and guiding clients through the housing application process.
About You
Required:
Bachelor's Degree in social work or related field, or at least four years of experience in health, nonprofit, or social services.
Experience with HMIS (Homeless Management Information System).
A current CA Class C Driver's License and proof of auto insurance.
Preferred:
Experience with Substance Use Disorder (SUD) services.
Shared experiences with potential clients, including histories of incarceration, homelessness, or mental illness.
Benefits
Competitive hourly wage of $28.00-$38.69.
Full-time, non-exempt position with regular hours Monday through Friday.
