About the Role
About You
- Must reside in Illinois.
- High School Diploma or equivalent.
- Minimum two (2) years prior experience working with community resources or health agencies.
- Valid driver's license, car insurance, and access to an automobile.
- Expected working knowledge using Microsoft Office Programs specifically Teams, Excel, PowerPoint, Outlook and Word.
- Must be able to work a 40-hour work week, Monday through Friday 8:00 AM to 5:00 PM.
- Bachelor’s degree in social work or related health field.
- Community Health Worker training and/or certificate.
- Familiarity with state Medicaid program guidelines.
- Experience engaging with Medicaid enrollees, including those with physical health and behavioral health needs.
