About the Role
As a Community Health Worker, you will be a vital link between patients, their communities, and care providers. Your responsibilities will include conducting in-home visits, telehealth consultations, collaborating with healthcare providers, assisting patients with insurance navigation, and ensuring culturally sensitive care. You will document patient interactions and maintain records to ensure continuity of care.
About You
Required:
- Associate degree or a combination of education and experience required; B.S or B.A. preferred.
- 2+ years in the healthcare arena or exposure to health care systems required.
- Experience as a Community Health Worker or equivalent supporting vulnerable populations required.
- CPR certification is required.
- Excellent verbal and written communication skills.
Preferred:
- CHW certification is a plus.
- Experience working with electronic medical records (EMR) and care management systems.
- Strong knowledge of HIPAA regulations.
