Oak and Cedar LLC is seeking a Community Inclusion Specialist to join their team in Sacramento, CA. The role involves empowering adults with developmental disabilities to thrive through meaningful connections and active participation in their communities.
About the Role
As a Community Inclusion Specialist, you will be responsible for promoting person-centered thinking, providing support in recreation, socialization, health and safety, and community inclusion. You will work at a 1:2 staff to client ratio, maintain schedules and documentation, and perform skills training as identified in each participant's Individual Service Plans. Additionally, you will transport participants in your personal vehicle and participate in the assessment process and data collection.
About You
Required:
- Must be 18 years of age.
- Possess a valid Driver’s license.
- Must have valid CPR/First Aid Card.
- Must receive clearance to work by the Department of Justice (Live scan).
- Must have reliable transportation.
